Purchase Order (PO) Form

Waves of Faith Purchase Order Form must be used prior to purchasing any item or to request a church card. PO forms should be submitted at least 7 days in advance for approval. Upon approval, purchase can be made. POs are reviewed on Wednesdays. You will receive a confirmation message when your PO is approved. Reimbursements not approved in advance may not be processed.

Counseling Request Form

First download and find a counselor on the TBA list.

Then, click here to complete a counseling request form.

Please complete the request form to receive up to six free counseling sessions. Approval process may take up to two weeks. Once approved: You pay for the first and fifth counseling session; Waves of Faith covers the second and sixth counseling session. The counselor donates the third and seventh session. TBA covers the fourth and eighth session.

Benevolence Request Form

Click here to complete the Benevolence Request form. The Benevolence Fund is to provide financial aid to an individual who is in need on an urgent basis. The Benevolence Fund may not be applicable for cases which need long-term financial support. The church has the right to adjust or to disapprove an applicant’s request and may consider helping other than monetary help.

Facility Rental Request Form

If your event is approved, you will receive a rental fee and contract agreement to sign that includes the facility usage guidelines. Each area and service has a set fee and requires a deposit. Rental is subject to facility availability and staff/cleaning availability. Rentals must be booked at least a month in advance.

Volunteer Background Check

Authorization may take up to two weeks. Failure to completely fill out all fields in the background form can result in a longer authorization. Once background check is cleared, the person will be added to the Approved Volunteer List.

Groceries are distributed on a first-come, first-served basis until supplies run out each day of operation. We advise that you call the Wesley Center at 817.625.8205 before arrival to confirm food availability. The address to the Wesley Center is: 3600 N. Crump St, Fort Worth, TX 76106.
